Community Forum to discuss the future of Long Term Care in Dane County

Join us for a community forum to discuss the future of Long Term Care in Dane County. Wisconsin is changing the way Long Term Care Services are provided, and this could mean a big change for residents in Madison and the surrounding area. Counties around the state are beginning the transition to Family Care and IRIS, two new programs that will change the way services are being provided to people with Developmental Disabilities, Physical Disabilities, and Frail Elders. Dane County will be making some big decisions soon.

Who: Seniors, People with Developmental Disabilities and
Physical Disabilities. People on a waiting list or receiving
waiver services.
Where: The Urban League Work Center
