Access to WI Medicaid Survey for Children 0-21

While Wisconsin Medicaid includes many services helpful to children with disabilities or special health care needs, some services are under-used and access to other services will be changing. Survival Coalition and Family Voices of Wisconsin are interested in your family’s experience with Medicaid prior authorization as well as your use and understanding of a Wisconsin Medicaid benefit for children called HealthCheck Other Services. Please share your family’s experience over the past year. The survey will take less than 5 minutes to complete and individual answers will remain confidential.
